Prescriptions and Medications
Order Repeat Medication
You can order your repeat medication in one of three ways:
1. Request online – Patient Access – please fill out the form below.
2. Nominated Chemist – Ask your nominated chemist to order it for you
3. Letter to the Practice – Write a letter to Clapham Family Practice (we do not accept telephone requests for repeat medications).
If you would like to save time and order all the medication you have on repeat at the same time rather than irregular times throughout the month please use the form below
Patient Access is a website and mobile app which gives you access to a wide range of GP services, such as ordering repeat prescriptions; booking appointments; and access to your medical records*. You can download the app for free in Google Play or the App Store. To register for Patient Access, please complete the form below:
*access to medical records is an advanced feature that requires additional authorisation and is for only 11 days, please only register for this if you require it as it will otherwise delay processing of your request.
When you next need to request an item on repeat count up all the tablets you have and fill in the form. Your doctor will issue a “one off” prescription of the tablets you need to synchronise all your medication to within a day or two.
Ask the practice pharmacist
If you have a query or would like more information about your medicines, and your community chemist has not been able to answer it, Clapham Family Practice employs a team of pharmacists who can help. They have been specially trained to prescribe medicines and issue prescriptions.
If you have questions about the medicines you are taking or have difficulties obtaining them, please book a telephone consult with our Practice Pharmacist Dilesh Vyas.
